The Institute of Medicine (IOM) (2003) report titled Unequal Treatment provided Congress with evidence of disproportionate rates of morbidity and mortality among racial and ethnic minorities. Major determinants of these poor health outcomes included sociodemographics, structural barriers, and service delivery barriers to accessing health care services experienced by racial and ethnic minorities. For various health care services, primary care providers are considered the first contact for point of entry to the health care system. Identifying and eliminating existing barriers to care at the point of entry for members of racial and ethnic minority groups is a matter of social justice for the social work profession. (Because the groups compared in the present analysis encompass both race [white Medicare beneficiaries] and ethnicity [Hispanic Medicare beneficiaries], findings, effects, and so forth are described as "racial and ethnic" throughout.) This is of particular importance in the age of managed care, when physicians are placed in the role of health care service "gatekeepers" (St. Peter, Reed, Kemper, & Blumenthal, 1999).
